Heart ultrasound

Echocardiography is an ultrasound examination of the heart. A probe sends out sound waves that are reflected to different degrees by the heart muscle, the valves and the blood. A computer assembles the echoes into moving images. Unlike X-rays or computed tomography, this involves no radiation exposure.

The standard method is called transthoracic echocardiography: the sound reaches the heart through the chest. You usually lie on your left side, and a gel ensures good contact between skin and probe. Through the gaps between the ribs and from below the breastbone, the heart is imaged from several directions. Doppler technology also shows the direction and speed of the blood flow, so that narrowed or leaking valves stand out.

The findings answer many questions at once. How large are the chambers and atria, how thick are the walls? How strongly does the left ventricle pump, expressed as the proportion of blood ejected, the ejection fraction? Do individual sections of the wall move less, for example after a heart attack? Fluid in the pericardium, blood clots in the heart and signs of increased pressure in the pulmonary circulation can also be detected. Common reasons for the examination are shortness of breath, swollen legs, a heart murmur, arrhythmias, long-standing high blood pressure or monitoring a known heart condition.

Some questions call for special forms. In transoesophageal echocardiography (TOE), a thin probe is passed down the oesophagus to a position behind the heart. This shows the valves, atria and clots in great detail; the examination is carried out on an empty stomach, with the throat numbed and often with a sedative. In a stress echo, the heart is scanned under physical exertion or medication-induced stress to detect reduced blood flow. If the image quality is not sufficient, for example in lung disease, an ultrasound contrast agent can help.

How much blood the left ventricle ejects with each beat is a key measure of heart failure and of how it responds to treatment.

Heart valves

Doppler measurements show whether a valve is narrowed or leaking and how severe this is. This determines the intervals between check-ups or whether a procedure is needed.

At the bedside

The device can be brought to the bed. In the intensive care unit, heart ultrasound thus helps to assess circulation and pumping function and to guide treatment.

The procedure

Ultrasound through the chest wall requires no injection and no special preparation. Allow some time for the discussion afterwards.

  1. Reason

    Symptoms, previous findings and the specific question are discussed, for example about pumping strength, a valve or a check-up after treatment.

  2. Positioning

    You undress to the waist, and three electrodes record an ECG at the same time. You usually lie on your left side with your left arm behind your head.

  3. Scanning

    The probe is placed at several points with gel and light pressure. Sometimes you will be asked to hold your breath briefly so that the lungs are not in the way.

  4. Measuring

    Chamber sizes, wall thicknesses, pumping function and blood flow are measured and the images saved, so that later examinations can be compared precisely.

  5. Findings

    Much can already be seen during the examination and is explained afterwards. It is then decided whether check-ups, treatment or further tests will follow.

Do I need to fast before a heart ultrasound?

Not for ultrasound through the chest wall. Eating, drinking and your usual medication are allowed. Only before a transoesophageal echo (TOE) should you have nothing to eat or drink for several hours, usually at least six.

Is the examination unpleasant?

Most people find it easy to tolerate. The gel is cool, and the probe is applied with some pressure, which can feel uncomfortable in places. Ultrasound through the chest wall has no side effects.

What does ejection fraction mean?

It indicates what proportion of the blood in the left ventricle is pumped out with each beat. In a healthy heart it is usually above 50 per cent; significantly lower values point to reduced pumping strength. The figure is always assessed together with the other findings.

When is a transoesophageal echo needed?

When the images through the chest wall are not sufficient or details are needed that show up better from behind. Typical reasons are a suspected inflammation of the heart valves or checking for clots in the atrium before a cardioversion, which restores the normal rhythm with an electric shock. Whether it is needed and where it takes place is discussed on an individual basis.

How often should a heart ultrasound be repeated?

It depends on the findings. A mild valve abnormality is often checked only every few years, a pronounced change or reduced pumping strength considerably more often. With normal findings and no symptoms, regular repetition is usually not necessary.

Does a heart ultrasound replace a cardiac catheter examination?

Not always. The ultrasound shows pumping strength, walls and valves, but not the coronary arteries themselves. For those, a CT scan of the heart or a cardiac catheter examination at a specialised heart centre may be considered.
